AGREEMENT TO TERMS
By accessing our website, booking a service, using our client portal, or receiving any service from TruePath Identity Hub, you agree to be bound by these Terms and Conditions in their entirety. If you do not agree to these terms please do not use our services. These Terms and Conditions constitute a legally binding agreement between you and TruePath Identity Hub.

SECTION 1 — WHO WE ARE
TruePath Identity Hub is a professional identity, testing, and screening services company located in Ridgeland, Mississippi. We provide DNA testing coordination, drug and alcohol screening, background check coordination, fingerprinting, nicotine and vaping detection, DOT compliance testing, and related services to individuals, families, employers, and organizations throughout Mississippi and the surrounding region.
TruePath Identity Hub serves as a professional collection and coordination facility. For laboratory-based services including DNA testing and laboratory-confirmed drug testing we partner with accredited third-party laboratory partners including DDC DNA Diagnostics Center for DNA services and certified laboratory partners for drug testing confirmation. Background check services are coordinated through our certified third-party screening partner Reveal Background Check. Results from these partners are subject to the terms and processing standards of those partners in addition to these Terms and Conditions.

SECTION 2 — SERVICES OFFERED
TruePath Identity Hub offers the following categories of services:

2.1 DNA Testing Services Including but not limited to paternity testing, maternity testing, siblingship testing, grandparentage testing, avuncular testing, zygosity twin testing, immigration DNA testing, court-admissible DNA testing, DNA profile STR, prenatal DNA testing, ancestry testing, lifestyle DNA testing, detection testing, and unusual sample testing. DNA testing services are coordinated through our AABB-accredited laboratory partner DDC DNA Diagnostics Center.

2.2 Drug and Alcohol Testing Including but not limited to DOT compliant drug testing, non-DOT drug screening, rapid drug screening, oral fluid saliva testing, alcohol testing, nicotine and vaping detection, and single panel specialty testing. Drug and alcohol testing services are performed using certified collection protocols and processed through certified laboratory partners where applicable.

2.3 Background Check Services Including but not limited to personal background checks, employment background checks, tenant screening, healthcare screening, DOT compliance screening, daycare and childcare screening, adoption agency screening, and CDL driver screening. Background check services are coordinated through our certified third-party screening partner.

2.4 Fingerprinting Services Professional fingerprinting for employment, licensing, and legal purposes.

2.5 Employer and Organization Screening Programs Including the TruePath SafeGuard Program employer membership plans, DOT random pool management, and custom employer screening packages.

2.6 Southern Paw Strand Labs Pet Testing Pet DNA testing, breed identification, pet health testing, animal drug detection, and pet nicotine detection services coordinated under the TruePath Identity Hub umbrella.

2.7 Client Portal and Subscription Services TruePath Portal Plus subscription plans providing ongoing access to results, documents, and services through the TruePath client portal.

SECTION 3 — APPOINTMENT AND BOOKING TERMS
3.1 Booking Required Most TruePath Identity Hub services require a scheduled appointment. Walk-in availability may be offered at our discretion but is not guaranteed. We recommend booking in advance to secure your preferred date and time.

3.2 Confirmation Your appointment is not confirmed until you receive a written confirmation from TruePath Identity Hub by email or through your client portal. A booking request does not guarantee an appointment slot.

3.3 Cancellation Policy Cancellations must be made at least 24 hours before your scheduled appointment. Cancellations made with less than 24 hours notice or failure to appear for a scheduled appointment without prior notice may result in a cancellation fee of up to $35.00 applied to your account. To cancel or reschedule contact us at support@truepathidentity.com or through your client portal.

3.4 Late Arrivals If you arrive more than 15 minutes late for your scheduled appointment TruePath Identity Hub reserves the right to reschedule your appointment. A rescheduling fee may apply at our discretion.

3.5 Required Documentation All clients must present a valid government-issued photo ID at the time of their appointment. For legal DNA testing all parties being tested must present valid government-issued photo ID. For DOT drug testing all drivers must present their CDL and employer authorization. For background check services a signed FCRA authorization form is required before any background check is processed. Failure to provide required documentation may result in your appointment being rescheduled.

3.6 Minor Clients For services involving minors a parent or legal guardian must be present for the entire appointment and must sign all required authorization forms on behalf of the minor. No minor may be tested without the written consent of a parent or legal guardian. For legal DNA testing involving minors a court order or signed consent from both parents or legal guardians may be required.

SECTION 4 — PAYMENT TERMS
4.1 Payment Due Payment is due in full at the time of service unless other arrangements have been made in writing with TruePath Identity Hub in advance. For SafeGuard Program members payment terms are governed by the SafeGuard Program Service Agreement.

4.2 Accepted Payment Methods TruePath Identity Hub accepts major credit cards, debit cards, and electronic payments processed through Square for in-person payments and Stripe for online payments. Additional payment methods may be available at our discretion.

4.3 Results Release Results will not be released until full payment has been received and cleared. TruePath Identity Hub reserves the right to withhold results until payment obligations are satisfied.

4.4 Returned Payments A returned check or failed electronic payment fee of $35.00 will be applied to your account for any payment returned by the bank or payment processor for any reason. After one returned payment all future payments must be made by certified funds, money order, or electronic payment only.

4.5 Refund Policy Once a sample has been collected and submitted to our laboratory partner for processing no refund will be issued. If testing cannot be completed due to an insufficient sample or sample integrity issue we will work with you to arrange recollection at no additional charge where possible. Refund requests for services not yet rendered must be submitted in writing to support@truepathidentity.com within 7 days of the original booking date.

4.6 Price Changes TruePath Identity Hub reserves the right to change service pricing at any time. Price changes will not affect services that have already been paid and scheduled. Current pricing is always available on our website at truepathidentity.com or by contacting us directly.

SECTION 5 — DNA TESTING TERMS
5.1 Chain of Custody Legal DNA testing requires strict chain of custody protocols as mandated by accrediting bodies and accepted by courts, legal authorities, USCIS, and embassies. All parties being tested for legal purposes must be present in person at TruePath Identity Hub for sample collection. Samples collected outside of our facility under chain of custody supervision are not accepted for legal testing purposes.

5.2 Personal Knowledge Testing DNA tests designated as personal knowledge or non-chain are for informational purposes only and are not admissible in court, legal proceedings, immigration petitions, or any official legal matter. Personal knowledge results are provided for private informational use only. TruePath Identity Hub expressly disclaims any liability for decisions made based on personal knowledge DNA test results.

5.3 Laboratory Processing All DNA samples are processed by our AABB-accredited laboratory partner DDC DNA Diagnostics Center. TruePath Identity Hub does not process DNA samples in-house. Turnaround times stated at booking are estimates provided by our laboratory partner and are not guaranteed. TruePath Identity Hub is not responsible for laboratory processing delays caused by sample volume, equipment issues, or other factors outside our control.

5.4 Accuracy DNA testing results produced by our AABB-accredited laboratory partner are highly accurate and meet or exceed industry standards. However no test is 100% infallible. In the extremely rare event of a disputed result we will work with our laboratory partner to review and if appropriate retest at no additional charge.

5.5 Confidentiality of Results DNA test results are strictly confidential. Results will only be released to the person who authorized and paid for the test or to an attorney or legal authority with proper documentation and authorization. TruePath Identity Hub will not release DNA results to any unauthorized third party under any circumstances.

5.6 Prenatal Testing Prenatal DNA testing including the Non-Invasive Prenatal Paternity test is performed on a maternal blood sample and is safe and non-invasive. However TruePath Identity Hub and our laboratory partner make no medical warranties regarding prenatal testing results. Prenatal testing clients are advised to consult with their healthcare provider. Prenatal results are for informational purposes only and do not constitute medical advice.

5.7 Unusual Sample Testing For unusual sample DNA testing including cigarette butts, toothbrushes, hair, and other non-standard samples TruePath Identity Hub makes no guarantee that sufficient DNA can be extracted from the submitted sample. If a sample does not contain sufficient DNA for testing a partial refund may be issued at our discretion after deducting processing and handling fees.

SECTION 6 — DRUG AND ALCOHOL TESTING TERMS
6.1 DOT Testing Compliance All DOT drug and alcohol testing is performed in strict compliance with 49 CFR Part 40 and applicable FMCSA, FAA, FTA, and other applicable federal agency regulations. TruePath Identity Hub is a certified DOT drug testing collection facility. Employers are responsible for ensuring their drug and alcohol testing programs comply with all applicable federal regulations. TruePath Identity Hub is not responsible for an employer's program compliance beyond the collection and testing services we provide.

6.2 Medical Review Officer All DOT drug test results are reviewed by a licensed Medical Review Officer before being reported to the employer. Non-negative results are not reported to employers until MRO review is complete.

6.3 Non-DOT Testing Non-DOT drug and alcohol testing is performed for employer, personal, and organizational use. Results from rapid on-site tests are presumptive and may require laboratory confirmation before any employment, legal, or clinical action is taken. TruePath Identity Hub expressly recommends laboratory confirmation testing for any non-negative rapid test result before any adverse action is taken.

6.4 Forensic Use Only Tests Certain drug testing products are designated Forensic Use Only and are not FDA cleared for use in employment or clinical decision making without laboratory confirmation. TruePath Identity Hub clearly identifies Forensic Use Only tests at the time of service. Clients who order Forensic Use Only tests acknowledge that results require laboratory confirmation before any legal, employment, or clinical action is taken.

6.5 Employer Responsibility Employers using TruePath Identity Hub drug and alcohol testing services are solely responsible for compliance with all applicable federal, state, and local laws governing workplace drug testing including but not limited to proper employee notification, signed consent forms, adverse action procedures, and FCRA compliance for background-related testing. TruePath Identity Hub is not liable for any employer's failure to comply with applicable drug testing laws and regulations.

6.6 Refusal to Test Any individual who refuses to provide a sample for a required drug or alcohol test will be documented as a refusal in accordance with applicable regulations. For DOT purposes a refusal to test is treated the same as a positive test result.

6.7 Specimen Integrity TruePath Identity Hub uses industry-standard collection procedures to ensure specimen integrity. In the event of a dilute, substituted, or adulterated specimen result TruePath Identity Hub will notify the appropriate parties in accordance with applicable regulations. A recollection fee may apply.

SECTION 7 — BACKGROUND CHECK TERMS
7.1 FCRA Compliance All background check services are conducted in compliance with the Fair Credit Reporting Act FCRA. Before any background check is processed all individuals being screened must sign a written FCRA-compliant authorization and disclosure form. TruePath Identity Hub will not process any background check without proper signed authorization.

7.2 Employer Obligations Employers using background check services for employment purposes are solely responsible for compliance with all FCRA adverse action procedures including providing the required pre-adverse action notice, a copy of the background check report, and a summary of FCRA rights to any applicant before taking any adverse employment action based on background check results. TruePath Identity Hub is not liable for an employer's failure to follow proper adverse action procedures.

7.3 Accuracy of Results Background check results are sourced from third-party databases, court records, and other public and private sources through our certified screening partner. TruePath Identity Hub makes no warranty regarding the completeness or absolute accuracy of background check results as these are dependent on the accuracy and completeness of source records. If an individual believes their background check results contain an error they have the right to dispute the information directly with our screening partner.

7.4 Permissible Purpose Background check services may only be used for lawful permissible purposes as defined by the FCRA including employment screening, tenant screening, personal safety purposes, and other lawful uses. Using background check results for any unlawful purpose is strictly prohibited. TruePath Identity Hub reserves the right to refuse background check services to any individual or organization we have reason to believe intends to use results for an unlawful purpose.

7.5 Retention of Records Clients are responsible for retaining signed FCRA authorization forms for a minimum of five years as required by applicable law. TruePath Identity Hub maintains records of background check orders in accordance with our data retention policy.

SECTION 8 — CONFIDENTIALITY AND PRIVACY
8.1 Confidentiality of All Results All test results, personal information, and records obtained through TruePath Identity Hub services are strictly confidential. TruePath Identity Hub will not share, sell, disclose, or distribute any client information or results to any third party without the express written consent of the authorized client except as required by law, court order, or applicable regulation.

8.2 Privacy Policy The collection, use, and storage of personal information provided to TruePath Identity Hub is governed by our Privacy Policy which is incorporated into these Terms and Conditions by reference. Our Privacy Policy is available at truepathidentity.com/privacy-policy.

8.3 Data Security TruePath Identity Hub uses industry-standard security measures to protect client data stored in our systems including encryption of sensitive personal information such as Social Security Numbers, date of birth, and test results. However no data transmission or storage system is completely secure and TruePath Identity Hub cannot guarantee the absolute security of information transmitted over the internet.

8.4 Third Party Partners Information shared with our third-party laboratory and screening partners is subject to those partners' privacy and confidentiality policies in addition to our own. TruePath Identity Hub only shares the minimum information necessary to process your requested service with our authorized partners.

SECTION 9 — CLIENT PORTAL AND SUBSCRIPTION TERMS
9.1 Portal Access The TruePath Identity Hub client portal is provided as a convenience for clients to access their results, upload documents, and manage their services. Portal access is subject to these Terms and Conditions and any additional terms presented within the portal.

9.2 Free Trial Access New clients receive 30 days of complimentary portal access upon creating their account. After 30 days a TruePath Portal Plus subscription is required to maintain portal access.

9.3 Subscription Plans TruePath Portal Plus subscription plans are available on a monthly basis at $9.99 per month, annual basis at $79.99 per year, or as a one-time Lifetime purchase at $199.99. Subscriptions are processed through Stripe and automatically renew unless cancelled before the renewal date.

9.4 Lifetime Access Limitations The TruePath Portal Plus Lifetime plan provides permanent access to all results and documents from services completed at or before the date of Lifetime plan purchase and all prior services. Results from new services ordered after the Lifetime plan purchase date require an active Monthly or Annual subscription or a Lifetime plan upgrade to access. Lifetime plan holders receive a 25% discount on any future Lifetime plan upgrade.

9.5 Fingerprint Reprint Packs Fingerprint reprint packs are available in increments of 5 reprints at $49.99, 10 reprints at $89.99, and 20 reprints at $159.99. Reprint packs for 5 and 10 reprints are valid for 12 months from purchase date. The 20 reprint pack is valid for 24 months from purchase date. Unused reprints expire at the end of the validity period and are non-refundable. Each reprint request is processed within 3 business days.

9.6 Account Security You are responsible for maintaining the confidentiality of your portal login credentials. TruePath Identity Hub is not responsible for any unauthorized access to your account resulting from your failure to safeguard your login information. Please notify us immediately at support@truepathidentity.com if you believe your account has been compromised.

9.7 Password Confirmation for Results For the security of your sensitive results certain portal actions including viewing DNA results and downloading completed test results require re-entry of your account password. This is a security measure designed to protect your confidential information.

SECTION 10 — EMPLOYER SAFEGUARD PROGRAM TERMS
Clients enrolled in the TruePath SafeGuard Program are subject to these Terms and Conditions as well as the terms of their executed SafeGuard Program Service Agreement. In the event of any conflict between these Terms and Conditions and the SafeGuard Program Service Agreement the terms of the SafeGuard Program Service Agreement shall govern for SafeGuard-specific matters.

SECTION 11 — LIMITATION OF LIABILITY
11.1 No Medical Advice TruePath Identity Hub is not a medical provider. Nothing provided by TruePath Identity Hub including test results, written communications, or verbal communications constitutes medical advice, diagnosis, or treatment. For any health-related concerns please consult a licensed healthcare provider.

11.2 No Legal Advice TruePath Identity Hub is not a law firm. Nothing provided by TruePath Identity Hub constitutes legal advice. For any legal matters related to testing results please consult a licensed attorney.

11.3 Limitation of Damages To the maximum extent permitted by applicable law TruePath Identity Hub's total liability to any client for any claim arising out of or related to our services shall not exceed the amount paid by the client for the specific service giving rise to the claim. TruePath Identity Hub shall not be liable for any indirect, incidental, consequential, special, or punitive damages arising out of or related to our services regardless of the cause of action and whether or not TruePath Identity Hub has been advised of the possibility of such damages.

11.4 Third Party Actions TruePath Identity Hub is not responsible for the actions, errors, omissions, or delays of our third-party laboratory partners, screening partners, or any other third-party service providers. While we carefully select our partners and hold them to high standards we cannot guarantee their performance and are not liable for outcomes resulting from third-party processing.

11.5 Force Majeure TruePath Identity Hub is not liable for any failure or delay in performing services due to circumstances beyond our reasonable control including but not limited to natural disasters, pandemics, government orders, laboratory closures, system outages, or other force majeure events.

SECTION 12 — INDEMNIFICATION
You agree to indemnify, defend, and hold harmless TruePath Identity Hub, its owner, employees, agents, and representatives from and against any claims, damages, losses, liabilities, costs, and expenses including reasonable attorneys' fees arising out of or related to your use of our services, your violation of these Terms and Conditions, your violation of any applicable law or regulation, or your use of test results in a manner inconsistent with these Terms and Conditions.

SECTION 13 — INTELLECTUAL PROPERTY
All content on the TruePath Identity Hub website including text, graphics, logos, images, service descriptions, and other materials is the property of TruePath Identity Hub and is protected by applicable copyright, trademark, and other intellectual property laws. You may not copy, reproduce, distribute, modify, or create derivative works from any TruePath Identity Hub content without our express written permission.

The TruePath SafeGuard Program name, logo, and all associated program materials are proprietary to TruePath Identity Hub. Unauthorized use of the SafeGuard Program name or materials is strictly prohibited.

SECTION 14 — GOVERNING LAW AND DISPUTE RESOLUTION
14.1 Governing Law These Terms and Conditions are governed by and construed in accordance with the laws of the State of Mississippi without regard to its conflict of law provisions.

14.2 Jurisdiction Any legal action or proceeding arising out of or related to these Terms and Conditions or our services shall be brought exclusively in the state or federal courts located in Madison County or Hinds County Mississippi. You consent to the personal jurisdiction of these courts.

14.3 Dispute Resolution Before initiating any legal action you agree to contact TruePath Identity Hub at support@truepathidentity.com and make a good faith effort to resolve any dispute informally. TruePath Identity Hub will respond to your dispute notice within 10 business days. If the dispute cannot be resolved informally within 30 days either party may pursue legal remedies as provided in these Terms and Conditions.

14.4 Waiver of Jury Trial To the maximum extent permitted by applicable law both parties waive their right to a jury trial for any claim arising out of or related to these Terms and Conditions or our services.

SECTION 15 — GENERAL PROVISIONS
15.1 Entire Agreement These Terms and Conditions together with our Privacy Policy and any executed service agreements constitute the entire agreement between you and TruePath Identity Hub regarding our services and supersede all prior agreements, understandings, and representations.

15.2 Severability  If any provision of these Terms and Conditions is found to be unenforceable or invalid by a court of competent jurisdiction that provision shall be modified to the minimum extent necessary to make it enforceable or if modification is not possible shall be severed from these Terms and Conditions. The remaining provisions shall continue in full force and effect.

15.3 Waiver TruePath Identity Hub's failure to enforce any provision of these Terms and Conditions shall not constitute a waiver of our right to enforce that provision in the future.

15.4 Assignment You may not assign or transfer your rights or obligations under these Terms and Conditions without our prior written consent. TruePath Identity Hub may assign these Terms and Conditions without restriction.

15.5 Updates to Terms TruePath Identity Hub reserves the right to update or modify these Terms and Conditions at any time. Updated Terms and Conditions will be posted on our website with a new effective date. Your continued use of our services after any update constitutes your acceptance of the revised Terms and Conditions. We encourage you to review these Terms and Conditions periodically.
